School Supply Giveaway WAITLIST Form

We have reached our capacity of the first 50 children; however, we are making a waitlist in case we have cancellations. If you would like to be added to the waitlist, fill out the information below, and we will notify you if we have a cancellation. Please complete one reservation form PER CHILD.
